Build, lead, and scale chronic disease medication management programs

Develop and maintain guideline directed medication management strategies and practice guidelines

Drive medication cost containment across commercial, Medicaid, and Medicare Advantage contracts

Monitor medication utilization trends and partner with finance and analytics on forecasting and budget planning
-
Support performance under value based and shared savings contracts
-
Partner with medical leadership to integrate medication insights into care delivery workflows
-
Collaborate with care management, operations, technology, and clinical analytics teams on medication strategies
-
Oversee and collaborate with external pharmacy and medication management vendors
-
Ensure vendor adherence to evidence based protocols, regulatory requirements, and contract expectations
-
Establish program metrics, monitor performance, and lead continuous improvement initiatives
-
Translate data and program outcomes into actionable insights for executive and clinical leaders
-
Role success measured by medication cost reduction, adherence to guideline directed therapy, chronic disease outcomes, and program growth
-
Minimum 10 plus years of experience in clinical pharmacy, chronic disease management, or healthcare leadership
-
Demonstrated experience managing medication strategies within value based or risk based arrangements
-
Strong financial acumen in cost management, forecasting, and performance improvement
-
Deep understanding of payer dynamics, population health, and evidence based medicine
-
Proven ability to lead cross functional initiatives and influence clinical and operational stakeholders
-
Advanced clinical degree such as PharmD, MD, or equivalent preferred
-
Experience partnering with analytics and technology teams and working with Medicare Advantage, Medicaid, or large commercial risk preferred
-
