Board Certified Internal Medicine or Family Medicine or Geriatric Medicine
New office location in Lady Lake, FL with an existing patient panel
Lower daily patient volume focused on comprehensive patient care
Outpatient-only schedule with no nights, weekends, holidays, or hospital responsibilities
Monday–Friday schedule
Provide care for the senior patient populations
Focus on prevention, quality outcomes, and evidence-based medicine
Manage acute and chronic conditions including diabetes, COPD, CHF, obesity, and cardiovascular disease
Team-based support including care coordination, pharmacy resources, and data tools
Opportunity to supervise Nurse Practitioners and Physician Assistants
Established patient base in a managed care setting
EMR-based practice environment
Seeking physicians with strong diagnostic and chronic disease management experience
Active Florida medical license
Minimum 3 years of outpatient primary care experience preferred
