ABOUT THE ROLE

  • We are seeking an experienced Internal Medicine Physician to join a mission-driven primary care practice serving older adults in underserved communities. You will provide comprehensive primary care both in-clinic and via telehealth while working alongside a dedicated care team focused on addressing social determinants of health.
  • Your impact: See the whole patient, not just their diagnosis. Work with embedded support specialists to address housing, nutrition, transportation, and community connection. Our integrated model has achieved a 15% reduction in hospital readmissions.

WHAT YOU WILL DO

  • Manage a patient panel capped at 750 (currently around 700) with 15–18 patients per day, allowing for comprehensive, unhurried visits of approximately 20 minutes per patient
  • Lead daily team huddles to align on patient care plans and priorities
  • Conduct full-spectrum primary care: in-person exams, telehealth visits, longitudinal care coordination
  • Collaborate with care team members (EMTs, social support specialists) on integrated patient plans
  • Manage patient communication and follow-ups; maintain close engagement throughout their health journey
  • Document visits and clinical notes in our EHR (Athena)
  • After a few months, transition to one day per week of remote/telehealth work as determined by clinic leadership
  • Provide on-call support: 2 evenings per month and one weekend per quarter
  • Liaise with emergency departments regarding patient history and continuity of care
REQUIREMENTS

  • Active Massachusetts medical license (MD or DO) with active medical license, or ability to obtain prior to start
  • Proven track record in primary care settings (newly minted graduates: not a fit for this role)
  • Clinical proficiency in evidence-based primary care
  • Comfort with EHR systems and digital health tools
  • Ability to connect authentically with patients from diverse backgrounds
  • Strong communication and documentation skills
  • Ethics and judgment to navigate complex, sensitive situations with confidentiality

PREFERRED QUALIFICATIONS

  • Prior experience treating older adults in underserved/complex social environments
  • Spanish language proficiency
  • Familiarity with telehealth and remote patient monitoring platforms
COMPENSATION & BENEFITS
  • $250,000–$290,000+ Base + Quarterly Performance Bonus
