WHAT YOU'LL DO
– Provide full-spectrum primary care, in clinic and via telehealth
– Lead daily team huddles on care plans and priorities
– Partner with care team members on integrated patient plans
– Manage patient communication and follow-ups
– Coordinate with emergency departments on continuity of care
– Document in Athena EHR
REQUIREMENTS
– Must live within a 10-mile radius of Worcester, MA
– Active Massachusetts NP license, or ability to obtain before start
– 3+ years of primary care experience as a practicing NP or PA
– New graduates will not be considered
– Strong evidence-based primary care skills
– Comfort with EHR and digital health tools
– Ability to connect with patients from diverse backgrounds
– Strong communication and documentation skills
PREFERRED
– Experience with older adults in complex social settings
– Spanish proficiency
– Telehealth or remote patient monitoring experience
