SNF Nurse Practitioner – Southern Massachusetts, Outside Providence

  • Join a large physician-owned multispecialty group as a Nurse Practitioner

  • Provide care for homebound patients and local skilled nursing facility (SNF) patients

  • Deliver proactive, efficient, and cost-effective managed care

  • Perform house calls and facilitate transitions from hospital/SNF to home

  • Reconcile medications, educate patients/caretakers, diagnose conditions, and order testing

  • Communicate with visiting nurse agencies, PCPs, specialists; coordinate care and referrals

  • Collaborate with other NPs and facility medical directors in SNF settings

  • Part-time schedule (32 hours per week) with competitive salary and benefits (health, malpractice, 401K, CME allowance, PTO)

  • Must be licensed in MA (RI license a plus), DEA certificate, and MA controlled substance registration

  • Seeking candidates with maturity, organizational skills, independence, flexibility, and problem-solving ability

  • Portuguese fluency is a plus, but not required

  • Vibrant location with easy access to city amenities and scenic coastal areas

  • 20 minutes from Providence, hour from Boston, and 30 minutes from Cape Cod
