  • Covers 13 SNF and ALF facilities in Polk County

  • Visit approximately 3 facilities per day

  • Monday – Friday schedule, no weekends

  • Highly autonomous role

  • Mentorship available as needed

  • “Round and go” model—leave once all patients are seen

  • Share weekend phone call coverage with 3 other providers

  • Compensation based on patients seen; estimated $120,000 – $150,000
