We’re seeking a caring and experienced Nurse Practitioner to provide high-quality primary care for an adult/senior population. Join a collaborative team focused on strong patient relationships and meaningful outcomes.
Responsibilities
- Provide comprehensive primary care under physician collaboration
- Assess, diagnose, and manage acute and chronic conditions
- Order/interpret diagnostics and create individualized care plans
- Coordinate care with specialists and document accurately in the EMR
Requirements
- Bilingual Spanish a plus
- Active NP licensure1–3+ years primary care experience preferred
- Strong communication, critical-thinking, and patient-relationship skills
- EMR proficiency and team-oriented mindset
