Nurse Practitioner – Primary Care
We’re seeking a caring and experienced Nurse Practitioner to provide high-quality primary care for an adult/senior population. Join a collaborative team focused on strong patient relationships, preventative care, and meaningful outcomes.
Responsibilities
• Provide comprehensive primary care in a collaborative clinical model
• Assess, diagnose, and manage acute and chronic conditions
• Order and interpret diagnostic studies; develop individualized treatment plans
• Coordinate care with specialists to ensure continuity
• Maintain accurate, detailed clinical documentation in the EMR
Requirements
• Active Nurse Practitioner licensure (or ability to obtain in Florida)
• 1–3+ years of primary care experience preferred
• Strong communication, clinical judgment, and patient relationship skills
• EMR proficiency and a team-oriented mindset
