– Provide routine and acute primary care visits
– Conduct annual wellness visits and HRAs
– Coordinate care with specialists and care teams
– Support population health and quality initiatives
– Patient education and care planning
– Active NP license and DEA
– 2 years of primary care outpatient experience
– HCC / ICD-10 / HRA familiarity
– Pay range $104,442.00 – $224,952.00
