Position Overview
- Full-time outpatient NP role
- Medicare patient population
- Independent practice with physician collaboration
- Dedicated Medical Scribe support
- Reports to Center Medical Director
Responsibilities
- Provide routine and acute primary care visits
- Conduct annual wellness visits and HRAs
- Coordinate care with specialists and care teams
- Support population health and quality initiatives
- Patient education and care planning
Requirements
- MSN, DNP, or NP Certificate Program
- Board Certified NP
- Active NP license and DEA
- 2 years of primary care outpatient experience
Preferred
- HCC / ICD-10 / HRA familiarity
