Primary Focus & Responsibilities
• Provide comprehensive inpatient hospital care
• Coordinate care with multidisciplinary teams
• Support safe, efficient discharge planning
• Focus on reducing readmissions through proactive patient management and communication
• Deliver high-quality, patient-centered outcomes
Qualifications
• Board Certified in Internal Medicine or Family Medicine
• Strong communicator and collaborator
• Commitment to quality, safety, and continuity of care
